2007 W-2 Information
State employee W-2s for Calendar Year 2007 were mailed on January 25, 2008.
If you have a question regarding your W-2, or need a replacement, please contact your Payroll or Human Resource section to obtain a W-2 request form.
All W-2 requests must be forwarded to the PHRST office from your Payroll or Human Resource section. For your protection, W-2s cannot be faxed.
Duplication of 2007 W-2s will begin on Friday, February 15th and no requests will be accepted before that date.
